On September 18, more than 40 community leaders came together for an in-person convening of Nevada County’s Community Health Improvement Project. This group first launched in March 2023 to guide Nevada County’s current Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) cycle, which sets shared priorities for building a healthier community.
The convening was an opportunity to reflect on progress since the CHIP was adopted in January 2025 and to hear updates from the three coalitions driving this work:
- Early Learning & Care Coalition, expanding opportunities for families and children.
- Immunization Coalition, working to increase childhood vaccination rates.
- Mobile Health Coalition, bringing care and services into schools, shelters, and community hubs.
Representatives from across sectors, including healthcare, education, social services, and community-based organizations, shared updates and identified opportunities to work together. Participating organizations included: 211 Connecting Point, Bright Futures for Youth, Chapa De Indian Health, Common Goals, Foothills Compassionate Care, Hospitality House, Interfaith Food Ministry, Nevada County HHSA (Public Health & Behavioral Health), Nevada County Pride, Nevada County Superintendent of Schools, Partnership HealthPlan, San Juan Ridge Community Center, Sierra Nevada Children’s Services, Sierra Nevada Memorial Hospital, Social Entrepreneurs Inc., Tahoe Forest Health System, Tahoe Truckee Unified School District, Western Sierra Medical Clinic, and What’s Up Wellness.
Two new community resources highlighted at the meeting reflect the work of our CHIP coalitions and the goals of the plan:
- nevcolpc.org, developed in partnership with the Early Learning & Care Coalition, is now a central hub for Universal Pre-K and early learning information. Families and providers can find up-to-date tools and guidance in one place.
- nevadacountyvaxfacts.com, created through the Immunization Coalition, is a one-stop site for vaccine schedules, vaccination locations, and community videos. We encourage partners to link to VaxFacts on their websites, share the flyer, and use it in their outreach so community members have a trusted place to turn for information.
Public Health is proud to provide leadership in convening partners and supporting cross-sector collaboration, but it’s the combined efforts of many organizations that make progress possible.
