  • Individuals experiencing Homelessness, Mental Health, Substance Use Disorder, Chronic Care, Veteran Services, Traumatic Brain Injury, Intellectual or Developmental Disability, HIV, Pregnancy, etc.

  • A CRC will meet with the Incarcerated Person within their first 3-5 days.

  • Incarcerated Persons can request referrals through the kiosk, a Correctional Officer, any jail medical staff, or directly to the CRC.

  • Mental health services, Enhanced Care Management, Inpatient and outpatient treatment facilities, Medical assisted treatment programs, housing support, food distribution programs, insurance benefits, employment services, education programs, veteran’s services, vital records and identification, and more
