2024 Community Health Assessment

The Community Health Assessment (CHA)  identifies key health needs and issues through comprehensive data collection and analysis, while the Community Health Improvement Plan (CHIP) will build off that assessment by developing long-term goals, strategies, and activities to address public health concerns in Nevada County. The CHA and CHIP will be used by the Nevada County Public Health Department and other governmental, education, and human service agencies, in collaboration with community partners, to set priorities and coordinate and target resources.

Through the Community Health Improvement (CHI) process, we will develop a 3 to 5-year plan that will serve as a road map for how we can increase healthy living and health equity throughout Nevada County.

Getting Involved

The CHI process is a collaborative process and each of us has a role in working to improve health and the quality of life in Nevada County.  We need your help! We want to develop a CHA and CHIP that include the voices of Nevada County’s diverse communities and individuals and that works to improve health for all our residents. There will be many opportunities for you to be involved during this process!
