24/7 Communicable Disease Reporting For Clinicians
The Division of Communicable Disease Control & Prevention responds to reports of communicable diseases in Nevada County. Physicians and health care providers are legally required by Title 17 of the California Code of Regulations to report suspected, lab-confirmed, and clinical diagnoses of specific diseases and conditions within specified time frames to the client’s county of residence. Confidential Morbidity Report (CMR) forms are for provider use to assist in reporting communicable diseases.

Animals often bite in response to being afraid or hurt. If you find a sick or injured animal, avoid handling the animal, especially with bare hands, and call animal control.

Report
Healthcare facility personnel are required to report all incidents of animal bites to the Nevada County Public Health Department and one of the appropriate Animal Control Agencies.

Notice to Health Care Providers and Others Responsible for Disease Reporting: California Code of Regulations, Title 17, Section 2500 and Section 2643.5 require that all diagnosed cases of AIDS and HIV as defined by CDC must be reported within seven (7) days to the Health Officer. For questions regarding HIV/AIDS reporting requirements or to begin the reporting process via phone you may contact the HIV Surveillance Coordinator at 530-265-1420.
For adult patients over 13 years of age at time of diagnosis, complete the Adult HIV/AIDS Case Report (ACRF). Instructions on how to fill out the ACRF may be found here.
For pediatric patients under 12 years of age at time of diagnosis, complete the Pediatric HIV/AIDS Confidential Case Report.
Mailing Instructions for Reporting HIV/AIDS
Double-envelope all HIV/AIDS-related materials.
Outer envelope (e.g. sender and/or recipient address or label) must have no references to HIV/AIDS.
Inner envelope shall be marked “Confidential,” sealed, and addressed to Cindy Key, HIV Surveillance.
All mail must be sent by traceable courier service only, such as FedEx or certified mail.
E-mailing case information containing personal identifiers is strictly prohibited. Do not e-mail, fax, or send by first-class mail.

Testing of ticks for B. burgdorferi, the agent of Lyme Disease, is generally not recommended due to the following:
- Results of tick testing should not be used for treatment decisions.
- Positive results showing that the tick contains a disease-causing organism do not necessarily mean that you have been infected.
- Negative results can lead to false assurance. You may have been unknowingly bitten by a different tick that was infected.
- Ticks can potentially transmit diseases other than Lyme Disease.
If you develop a rash or fever within several weeks of removing a tick or have any other concerns, see your doctor. Be sure to tell the doctor about your recent tick bite, when the bite occurred, and where you most likely acquired the tick.
