Site Plan
Provide a detailed site plan showing where proposed work will be done.
Certificate of Insurance
Minimum required limits are $1,000,000 General Liability and $2,000,000 General Aggregate with the County of Nevada, Public Works named as the additional insured.
Traffic Control Plan
Provide an engineered traffic control plan for the project or a standard MUTCD.
*Road closure requests require pre-approval by the Public Works Road Commissioner.

Provide an engineered traffic control plan for the project or a standard MUTCD. If a full road closure or intermittent lane closure will be requested, please also provide a Security Support Letter (CHP or Sheriff).
