The Electronic Health Record forms listed on the right hand side of the page are available to our organizational providers to assist them in managing this information. Contact the Behavioral Health Billing/Claims Department if you are unable to locate the information your organization needs.
Admission Notification and Authorization Request
If you are an Inpatient Psychiatric Hospital or PHF and you would like to notify Nevada County of a beneficiary admission or to request authorization, please contact (866) 449-2737 or CAReviews@Kepro.com.
